Clinic Calendar / Dates To Remember

The hyperlinks on this page will take you to the Clinic Handbook and other important forms and documents used in your clinical practicum.

Daily Clock Hour Form (Appendix D-3, updated March '06)  This form should be completed daily and the clinical supervisor for that day should initial in the appropriate column.  Please note that you need to note at the top the number of pages (i.e. 1 of 3, 2 of 3, 3 of 3, etc.).

Cumulative Clock Hour Form (Appendix D-4)  This form is used to compile and submit the hours noted on your Daily Clock Hour Form on a monthly basis.  It is also set up to serve as a semester composite and the final composite.  You will submit one monthly, two at the end of the semester (one for the month and one for the semester), and three prior to graduation at the graduate level (one for the month, one for the semester, and one for the final).  You may fill in the hours and the program will total the hours for you, then print!  You will not, however, be able to save the file.

All clinical practicum students are required to read the clinic handbook, and there are several pages that you are required to print out, sign, and submit to the Clinic Director prior to beginning practicum.  The Clinic Director will inform you of the required signature pages.

Student OBSERVERS:

Forms needed for observation are D1 and D2.  Please click below and print the one you need.
